Filing of Living Will
- Any adult citizen can file the Living Will instruction. He should be of sound health and mind preferably certified so by a family doctor or consulting physician.
- It can be prepared on a normal Bond Paper or Ledger Paper.
- You can use the format attached herewith or you may get one made by your Lawyer.
- This may be printed/typed and two of your friends (independent) will sign as your witness.
- After this is done, the document may be certified by a notary or a gazetted officer.
- While preparing the Living Will instruction you have to authorize one more guardian/authorized to inform the treating physician to make him aware of who is authorized to confirm the directives.
- Once this is done a copy of the document be delivered to the Custodian appointed in your Municipal Corporation, City Corporation or any local body as the Government directs.
- On completion of this, documents and certified copies be given to the Custodian, executor, guardian, consulting physician, and other individuals.
- Once the doctor confirms the medical condition of the patient the guardian may initiate action to ensure that the concerned people are informed too.
